Faith Formation Registration

Register your family for the 2026–2027 program year. Faith Formation prepares students to receive First Holy Communion (Grade 2) and, in the later years, Confirmation (Grade 8).

Grades 1–3Family Catechesis — families attend together, once a month in person. Work will be completed at home with families.
Grades 4–6Sunday mornings — in person classes.
Grades 7–8Monday evenings — in person classes.

Faith Formation Policies and Procedures *

    Photo Release *

    The Faith Formation Program of St. Elizabeth Parish may wish to use a picture of your child taken at CCD or at a CCD-related activity. The parish reserves the right to determine which pictures are used and how long they remain in use.

    Circle of Grace *

    The goal of the Circle of Grace program is to educate and empower children and young people to actively participate in a safe environment for themselves and others.

    Pick-Up Authorization

    List any adults besides yourself who may pick up your child(ren). Each authorized person must be at least 16 years of age. Authorized individuals must pick up the child in person and may be asked to show identification. Children will not be released to anyone who cannot provide acceptable identification.

    You, as the enrolling parent or guardian, are always authorized — there is no need to list yourself. Leave this blank if no one else will be picking up.

    Registration Payment

    Note: your registration is not complete until payment is received. Fees are paid by cash or check made out to “St. Elizabeth Church.” Bring or mail your payment to the CCD office. (Online payment may be added later.)
